Coverage becomes effective on the earliest of the following dates:
- The date FCAM Se. notify CFOLDF by email that the individual would like coverage;
- The date we receive the completed application and payment if the individual enrolls directly with us; or
- The date the individual submits their application and payment through the COFLDF website.
The fee is $300
