Fire Chiefs needing help, please fill out the form below:

Notification is made to the program coordinators as well as the FCAM secretary.
A program coordinator will contact the mentee and/or the District Director to gather additional information.
Program Coordinators match circumstances and demographics, and assign mentor.
The mentor calls the Fire Chief, who requested assistance for initial contact.
